Dear Valued Client of Peace of Mind Mental Health Services,
Please note that this clinic has closed. I am no longer offering in-person or virtual services. I have very much enjoyed working with you and hope that our work has improved the quality of your life. Your strength and resilience will remain an inspiration.
Since the beginning of your service with Peace of Mind Mental Health Services, your personal health information has been collected, used and stored in Jane Practice Management Software which has been an effective tool to provide you quality service while following the Personal Health Information Privacy Act. As of December 31, 2023, your personal health information and records will no longer be stored in Jane Practice Management. Your information will be exported as a PDF file and safely exported to an encrypted storage device. This device will be stored in a locked cabinet for 10 years from the date of last entry or, if you are a minor, information will be stored for a period of 10 years after your 18th birthday.
Furthermore, all data in Peace of Mind Mental Health Services’ Jane Practice Management account will be made inaccessible but will be saved on their secure servers. This helps protect your information in the event original files are lost or destroyed.
Should you require access to your records within the stated storage period, you will be able to place a request via email at Melissa@YourPeaceofMind.ca. As per company policy, there will continue to be fees for accessing copies of records. Fees to access personal records will be charged in 15-minute increments at the rate of $150.00 per hour.
